Yaroslav M. Kostyshyn

Born on June 19, 1981, in the city of Ivano-Frankivsk. In 1998, he graduated from the general education school in the village of Zahvizzia, Tysmenytsia district, Ivano-Frankivsk region. Since 1998, he has been in the Armed Forces of Ukraine. In 2002, he graduated from the Lviv Military Institute named after Hetman Petro Sahaidachny, part of the National University "Lviv Polytechnic," with a degree in "Automobiles and Automotive Industry." Since 2002, he served in the 649th aviation ammunition and munitions supply unit of the Air Force of the Armed Forces of Ukraine (military unit A3013, village of Hruzevytsia, Khmelnytskyi district, Khmelnytskyi region). In September 2014, Major Kostiushyn voluntarily joined the combined unit "Wild Duck" of the Air Force of the Armed Forces of Ukraine, which was to serve in the combat zone in eastern Ukraine. From September 25, 2014, he participated in the anti-terrorist operation in eastern Ukraine. On October 27, 2014, Major Kostiushyn was killed by shrapnel during an artillery shelling by Russian-terrorist groups of the positions of Ukrainian troops near the city of Avdiivka, Donetsk region, in the area of the Donetsk International Airport. On October 29, 2014, he was buried in the cemetery of the village of Tomashivka, Yarmolynetskyi district, Khmelnytskyi region. On October 16, 2015, a memorial plaque was unveiled in the village of Zahvizzia, Tysmenytsia district, on the facade of the general education school (5a Pidhirka Street), where Yaroslav Kostiushyn studied.
