      • Born on October 15, 1980, in the city of Sokal, Lviv region. In 1995, he graduated from the 9th grade of secondary school No. 5 in Sokal. In February 2015, he was mobilized into the Armed Forces of Ukraine. He served in the 53rd Separate Mechanized Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit V0927, in the cities of Severodonetsk/Lysychansk, Luhansk region). Since 2015, he participated in the anti-terrorist operation in eastern Ukraine. On February 27, 2016, soldier Kozachuk was killed while performing a combat mission. On March 5, 2016, he was buried in the cemetery of the city of Sokal, Lviv region.
