- Born on July 24, 1980, in the city of Sumy. In 1996, he graduated from the 9th grade of a general education school in Sumy, and in 1998, he completed vocational technical school No. 2 named after Hero of the Soviet Union Roychenko O.O. in Sumy (now the State Vocational and Technical Educational Institution "Sumy Center for Vocational and Technical Education"). In the spring of 2014, he voluntarily joined the ranks of the special forces battalion (reserve battalion) "Donbas" of the Northern Operational Territorial Association of the National Guard of Ukraine (military unit 3027). From the summer of 2014, he participated in the anti-terrorist operation in eastern Ukraine. Call sign "Art." On August 20, 2014, during the evacuation of the wounded from the city of Ilovaisk in the Donetsk region, a "Toyota" minibus, which contained one serviceman of the Armed Forces of Ukraine and eight fighters of the "Donbas" battalion, including reserve soldier Kuzyakov, was ambushed by two platoons of regular troops of the Armed Forces of the Russian Federation near the village of Hrabske in the Amvrosiivskyi district of the Donetsk region. In the first minutes of the battle, Artem Kuzyakov was killed. Also in this battle, reserve soldier Kupishinov ("Romanian"), reserve sergeant Utkyn ("Wizard"), and reserve soldier Nikolenko ("Karat") were killed.
