- Born on June 15, 1985, in the city of Székesfehérvár, People's Republic of Hungary (now Fejér County, Hungary) in a military family. In 1986, he moved with his family to the city of Novograd-Volynskyi in the Zhytomyr region. From 1992 to 1995, he studied at general education school No. 2 in Novograd-Volynskyi, and from 1995, at general education school No. 7 in Novograd-Volynskyi. In 2002, he graduated from general education school No. 7 in Novograd-Volynskyi, in 2006 from Vinnytsia Medical College, and in 2007 from the National Pharmaceutical University (Kharkiv) with a degree in "standardization and certification of medical products." In 2006, after graduating from Vinnytsia Medical College, he was awarded the title of "lieutenant of the medical service." He has been in the Armed Forces of Ukraine since 2007. He served as the deputy commander of the medical company, and as a hospital ward doctor of the medical company of the 30th Separate Guards Mechanized Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit A0409, Novograd-Volynskyi, Zhytomyr region). Since the spring of 2014, he served in Mykolaiv and Kherson regions at the administrative border with the occupied Autonomous Republic of Crimea. Since May 2014, he participated in the anti-terrorist operation in eastern Ukraine. He performed the duties of the head of the medical point of one of the units of the 30th Separate Mechanized Brigade. Thanks to the efforts of Captain of the Medical Service Herasymchuk, 32 wounded soldiers of the 30th Separate Mechanized Brigade, three soldiers of the Aidar battalion, and three police officers were evacuated from the battlefield and checkpoints. On May 22, 2014, the brigade's unit was ambushed near the city of Rubizhne in the Luhansk region. During the battle, two servicemen were killed, and six were injured. Captain of the Medical Service Herasymchuk, along with comrades who organized medical support for the convoy, provided medical assistance, applied tourniquets, performed dressings, and transported the wounded to the local hospital in Rubizhne. When they delivered the wounded comrades to the district hospital and wanted to return to their unit, it turned out that the city was blocked by militants. They had to spend the night in Rubizhne, in an unknown city. Thanks to the ingenuity of Captain of the Medical Service Herasymchuk, the servicemen spent the night safely, and later they not only managed to leave but also evacuated the wounded from the city and transported them by medical helicopter to the hospital. On July 8, 2014, the base camp of the 2nd battalion tactical group in the area of the village of Vesela Hora in the Slovianoserbsk district of the Luhansk region was subjected to artillery shelling by illegal armed groups. One of the artillery shells exploded above the tent where the medical point personnel lived. As a result of the explosion, Captain of the Medical Service Herasymchuk received a severe shrapnel wound to the head, from which he died. On July 10, 2014, he was buried at the military memorial cemetery (Chekhov Street) in Novograd-Volynskyi, Zhytomyr region. Military ranks: - lieutenant of the medical service (2006), - senior lieutenant of the medical service (2009), - captain of the medical service (2012), - major of the medical service (posthumously on 22.07.2015).
