- Born on October 31, 1994, in the village of Zastenki, Dubrovinsky District, Vitebsk Region, Republic of Belarus. After some time, the family moved to the village of Kurozvany, Hoshchansky District, Rivne Region. In 2010, he graduated from the general education school in the village of Kurozvany, Hoshchansky District. From August 2013, he served under contract in the 30th Separate Guards Mechanized Novograd-Volynskyi Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit A0409, city of Novograd-Volynskyi, Zhytomyr Region). From the summer of 2014, he participated in the anti-terrorist operation in eastern Ukraine. He was involved in the battles for the mound of Savur-Mohyla in the Shakhtarsky District of Donetsk Region. On May 7, 2015, soldier Kozachonok was killed as a result of a mortar shelling by Russian-terrorist groups targeting the positions of Ukrainian military in the area of the urban-type settlement of Luhanske in the Artemivsk (now Bakhmut) District of Donetsk Region. On May 10, 2015, he was buried in the cemetery of the village of Kurozvany, Hoshchansky District, Rivne Region. On September 20, 2015, a memorial plaque was unveiled in the village of Kurozvany on the facade of the general education school (Dmytro Kozachonok Street, 7), where Dmytro Kozachonok studied.Book of Remembrance for Ukraine - Ukrainian memorial
