      • Born on July 31, 1994, in the city of Bila Tserkva, Kyiv region. In 2009, he graduated from the 9th grade of the educational institution "First Bila Tserkva Gymnasium - School of the I degree" in Bila Tserkva, and in 2013 - from the Bila Tserkva Higher Vocational School of Construction and Service. From March 26, 2013, he served under contract in the 72nd Separate Mechanized Brigade of the Operational Command "North" of the Ground Forces of the Armed Forces of Ukraine (military unit A2167, city of Bila Tserkva, Kyiv region). He participated in the anti-terrorist operation in eastern Ukraine. On November 29, 2016, Junior Sergeant Slomchynskyi was killed by shrapnel wounds he received during a mortar shelling by Russian-terrorist groups of the positions of Ukrainian troops near the village of Kamianka in the Yasynuvata district of Donetsk region. On December 1, 2016, he was buried at the Alley of Glory in the "Sukhyi Yar" cemetery in Bila Tserkva. On December 6, 2017, a memorial plaque was unveiled in the city of Bila Tserkva on the facade of the First Bila Tserkva Gymnasium (Yaroslav Mudryi Street, 54), where Maksym Slomchynskyi studied.
