- Born on December 16, 1982, in the village of Tsybulivka, Trostyanets district, Vinnytsia region. In 1999, he graduated from the general education school in the village of Tsybulivka, then from Komarhorod Professional Technical School No. 39 (now Komarhorod Higher Professional School - village of Komarhorod, Tomashpil district, Vinnytsia region). From 2001 to 2002, he served in the military in a unit of the Air Defense Forces of the Armed Forces of Ukraine (city of Yevpatoria, Autonomous Republic of Crimea). From 2004 to 2006, he worked as a mechanic, then as a locksmith at a local agricultural enterprise, and in 2009-2010, he worked as a security guard at the company "Kyivmiskbud" (city of Kyiv). From 2010 to 2014, he engaged in personal subsidiary farming in his native village. On August 13, 2014, he was mobilized into the Armed Forces of Ukraine. He served as a service number in the 44th Separate Artillery Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit field post V1428, city of Ternopil). Since 2014, he participated in the anti-terrorist operation in eastern Ukraine. On February 11, 2015, soldier Melnyk was killed by a shrapnel wound to the head during shelling of the brigade's positions in the area of the city of Dzerzhynsk (now Toretsk) in Donetsk region near the city of Horlivka, Donetsk region. On February 14, 2015, he was buried in the New Cemetery of the village of Tsybulivka, Trostyanets district, Vinnytsia region.Book of Remembrance for Ukraine - Ukrainian memorial
