- Born on March 4, 1982, in the city of Moscow (now the Russian Federation) to a military family. After some time, the family moved to the German Democratic Republic. Then to the city of Zhytomyr. From 1989, he studied at general education school No. 26 in Zhytomyr, which he graduated from in 1999. Since 1999, he has been in the Armed Forces of Ukraine. In the early 2000s, he graduated with honors from the Kharkiv Guards Order of the Red Banner Tank Forces Institute named after the Verkhovna Rada of Ukraine National Technical University "Kharkiv Polytechnic Institute." He served in the 30th Separate Guards Mechanized Novograd-Volynskyi Rivne Order of the Red Banner and Suvorov Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit A0409, city of Novograd-Volynskyi, Zhytomyr region). Since the spring of 2014, he participated in the anti-terrorist operation in eastern Ukraine. Call sign "Azimuth." He participated in battles in the area of the village of Stepanyvka and the mound of Savur-Mohyla in the Shakhtarsk district, the city of Debaltseve in Donetsk region. On October 21, 2015, while performing official duties at the location of the unit in the area of the anti-terrorist operation, Major Yermak sustained multiple injuries as a result of a traffic accident and was evacuated to the intensive care unit of the Central District Hospital in the city of Artemivsk (now Bakhmut), Donetsk region. On October 23, 2015, Major Yermak died from his injuries in the ambulance on the way to the Military Medical Clinical Center of the Northern Region (city of Kharkiv). He was buried in the cemetery of the city of Novograd-Volynskyi, Zhytomyr region. On December 5, 2016, in the city of Zhytomyr, a memorial plaque was unveiled on the facade of general education school No. 26 (59 Myru Avenue), where Oleg Yermak studied.Book of Remembrance for Ukraine - Ukrainian memorial
