- Born on April 25, 1987, in the city of Zhovti Vody, Dnipropetrovsk region. In 2003, he graduated from the 9th grade of the communal institution "Zhovtovodskyi Educational Complex No. 6 'Perspektiva'," and in 2005, he graduated from Vocational School No. 70 in Zhovti Vody with a specialty in "electric gas welding." He worked as an electric gas welder. He served his mandatory military service in the Armed Forces of Ukraine. In April 2015, he was mobilized into the Armed Forces of Ukraine. He served as a commander of a combat vehicle - squad commander of the 2nd platoon of the 2nd motorized infantry company of the 40th separate artillery brigade of the Ground Forces of the Armed Forces of Ukraine (military unit V3500, city of Pervomaisk, Mykolaiv region). Since the summer of 2015, he participated in the anti-terrorist operation in eastern Ukraine. On August 26, 2015, Senior Soldier Matlak was killed by a fatal wound as a result of shelling from the Grad multiple rocket launcher system near the village of Prokhorivka in the Volnovakha district of Donetsk region when a shell hit his dugout. Four other servicemen were killed along with him. On September 4, 2015, he was buried in the city cemetery of Zhovti Vody.Book of Remembrance for Ukraine - Ukrainian memorial
