  Born on April 21, 1974, in the city of Mykolaiv. On April 23, 2014, he was mobilized to the 79th Separate Airborne Brigade of the High Mobility Airborne Forces of the Armed Forces of Ukraine (military unit A0224, city of Mykolaiv). Since the spring of 2014, he participated in the anti-terrorist operation in eastern Ukraine. On July 13, 2014, Sergeant Kostiushko was killed at 08:00 during the shelling of a Ukrainian military checkpoint by Russian-terrorist groups near the village of Porichchia in the Krasnodon district (since 2016 - Sorokynskyi) of Luhansk region. Senior Sergeant Yeroshenko was killed along with him. On July 16, 2014, he was buried in the cemetery of the city of Mykolaiv.
