- Born on July 5, 1995, in the village of Piskiv, Kostopil District, Rivne Region. After some time, the family moved to the city of Kostopil, Rivne Region. He graduated from the 9th grade of general education school No. 3 in Kostopil, and in 2012 he graduated from the Sarny Professional Agricultural Lyceum, obtaining the profession of "Tractor driver-machine operator in agricultural production, locksmith-repairman." He worked as a locksmith-plumber in a multi-industry housing and communal services enterprise in the city of Kostopil. In May 2016, he was called up by the Kostopil District Military Commissariat of Rivne Region for military service under contract in the Armed Forces of Ukraine. He served in the 501st Separate Battalion of Marine Corps of the 36th Separate Marine Brigade of the Naval Forces of the Armed Forces of Ukraine (military unit A1965, city of Berdyansk, Zaporizhzhia Region). Since the summer of 2016, he participated in the anti-terrorist operation in eastern Ukraine. On March 17, 2017, sailor Kondratiuk was killed during a mortar shelling by Russian-terrorist groups of the observation post of Ukrainian troops in the area of the village of Vodiane, Volnovakha District, Donetsk Region. Sergeant Halaychuk was killed alongside him. On March 21, 2017, he was buried at the Alley of Heroes of the Anti-Terrorist Operation at the New Cemetery in the city of Kostopil.Book of Remembrance for Ukraine - Ukrainian memorial
