- Born on September 20, 1984, in the city of Ivano-Frankivsk. Graduated from general education school-lyceum No. 23 in Ivano-Frankivsk, and in 2009 completed an internship at the Ivano-Frankivsk National Medical University. In the winter of 2013-2014, he was at Maidan Nezalezhnosti in Kyiv, where he provided medical assistance to sick and injured participants of the Revolution of Dignity. Since December 2014, he served in the 1st Separate Medical Company named after Pirogov of the Northern Territorial Kyiv Command of the National Guard of Ukraine (military unit 3001, Kyiv). From January 6, 2015, he participated in the anti-terrorist operation in eastern Ukraine. On January 12, 2015, the medical brigade of the 1st Separate Medical Company named after Pirogov went to the combat zone to evacuate the wounded. While providing medical assistance to an injured soldier, reserve medical service junior lieutenant Chomko received three gunshot wounds, as a result of which he died in a hospital in the city of Artemivsk (now Bakhmut) in Donetsk Oblast. He was buried on January 16, 2015, at the cemetery of the village of Krykhivtsi in the Ivano-Frankivsk City Council of Ivano-Frankivsk Oblast. In the city of Ivano-Frankivsk, on May 27, 2015, a memorial plaque was opened in his honor on the facade of the general education school-lyceum No. 23 (Mazepa Street, 90), and on October 14, 2015, another plaque was unveiled on the facade of the main building of the Ivano-Frankivsk National Medical University (Halytska Street, 2), where Yaroslav Chomko studied.Book of Remembrance for Ukraine - Ukrainian memorial
