Yevhen V. Kostiuk (Shlotsyk)

Born on August 28, 1996, in the city of Vinnytsia. In 2009, he graduated from the 6th grade of the general education school-gymnasium No. 23 in Vinnytsia, and in 2012 - from the 9th grade of general education school No. 11 in Vinnytsia. Since 2012, he studied at the Higher Vocational School No. 11 in Vinnytsia. In the winter of 2013-2014, he was at Maidan Nezalezhnosti in Kyiv, where he was an active participant in the Revolution of Dignity. In the summer of 2014, he volunteered to join the UNSO battalion (Ukrainian National Self-Defense). He underwent military training at the UNSO camp. Since December 2014, he participated in the anti-terrorist operation in eastern Ukraine. He was involved in the battles for the village of Pisky in the Yasynuvata district of Donetsk region. In February 2016, he joined the ranks of the 1st Separate Assault Company of the Volunteer Ukrainian Corps "Right Sector." On May 29, 2016, Yevhen Kostiuk sustained shrapnel wounds to the head as a result of an explosion from a "tripwire" while carrying out a combat mission near the "Putilivska" mine ("Butivka-Donetsk") in the Donetsk area. The wounded soldier was evacuated to the intensive care unit of the Mechnikov Regional Clinical Hospital in Dnipro, where doctors fought for his life for three days. On June 2, 2016, without regaining consciousness, Yevhen Kostiuk died from his injuries. On June 4, 2016, he was buried in the cemetery of the village of Luka-Meleshkivska in the Vinnytsia district of Vinnytsia region. On May 25, 2017, a memorial plaque was unveiled in the city of Vinnytsia on the facade of general education school No. 11 (Taras Sych Street, 38), where Yevhen Kostiuk studied.
