- Born on March 13, 1987, in the village of Hlushyn, Brody district, Lviv region. Graduated from a general education school. Mobilized into the Armed Forces of Ukraine. Served as a rifleman in the 54th Separate Mechanized Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit field post V2970, city of Artemivsk (since 2016 - Bakhmut), Donetsk region). Participated in the anti-terrorist operation in eastern Ukraine. On September 10, 2015, at a checkpoint near the village of Troitske in the Popasna district of Luhansk region, a group of servicemen, including soldier Krutyak, was blown up by a "tripwire" with an OZM-72 mine set by terrorists. As a result of the explosion, soldier Yosyp Krutyak was killed, and 5 servicemen were injured, one of whom, soldier Oleg Kuksa, died the same day from his injuries in a hospital in the city of Artemivsk (since 2016 - Bakhmut), Donetsk region.
