      • Born on January 12, 1979, in the village of Lisovody, Horodok district, Khmelnytskyi region. Graduated from the general education school of the village of Lisovody. Served mandatory military service in the Armed Forces of Ukraine. Worked as a mechanic at the Private Enterprise "Agrarian Company 2004" (village of Popivtsi, Volochysk district, Khmelnytskyi region). In mid-August 2014, mobilized into the Armed Forces of Ukraine. Served as a senior mechanic-driver in the 128th Separate Mountain Assault Brigade of the Ground Forces of the Armed Forces of Ukraine (military unit A1778 (field post V4673), city of Mukacheve, Zakarpattia region). Since the fall of 2014, participated in the anti-terrorist operation in eastern Ukraine. Participant in the battles for the city of Debaltseve, Donetsk region. On February 17, 2015, Junior Sergeant Ostiak was killed in battle in the area of the city of Debaltseve, Donetsk region, as a result of a mortar shelling. On February 27, 2015, he was buried in the cemetery of the village of Lisovody, Horodok district, Khmelnytskyi region.
